An AIM facilitator is assigned to each clinic within a collaborative to help participants with setting aims, collecting measurements, reporting, holding meetings and team development work pre, during and post collaboratives. Facilitators engage and support teams in their ongoing development of improvement capacity, and implementing improvement principles.
The facilitator possesses a unique skill set including knowledge of key content areas like: the Model for Improvement; Plan- Do- Study- Act cycles, process/flow mapping; measurement application; inter-professional collaboration; and, decision support tools and resources. In addition, they support team building, manage conflict and have knowledge of local health care environments.
As well, clinics can access other resources for assistance with spreadsheets, data collection, panel/caseload identification and support for situations where team issues are a barrier to success. Alberta AIM faculty is also available to participants and includes both physicians and non-physicians that provide information sharing and problem solving to support AIM.
